Dr Jocelyn Charles: Founder of OPCC in 2020 and Chair through 2021
Dr. Jocelyn Charles is a family physician in the Sunnybrook Academic Family Health Team, the Primary Care Clinical Co-Lead for North Toronto and Chief of the Veterans Program at Sunnybrook Health Sciences Centre. She has held many leadership roles including Physician Advisor, Primary Care Strategy for the Toronto Central LHIN, Chief of the Sunnybrook Department of Family & Community Medicine, Chair of the Sunnybrook Medical Advisory Committee, Chair of the Ontario Primary Care Council, a director on the Ontario College of Family Physicians Board of Directors. She is a Professor in the Department of Family and Community Medicine at the University of Toronto. Her clinical and academic work has been focused on developing patient-centred models of care to meet the needs of patients with complex needs both in the community and in long term care. Together with system partners, she has worked towards sustained equitable system improvements in health care delivery to all, particularly more vulnerable people in our society, using quality improvement methodology. She was recognized by the College of Family Physicians of Canada as the Family Physician of the Year for Ontario in 2019.
Dr Gord Schacter: Chair, October 2021-April 2023
Dr. Schacter has been practicing comprehensive family medicine in London for 30 years. He is the Regional Clinical Lead for Primary Care at Ontario Health West and Clinical Lead for Primary Care with the Middlesex London Ontario Health Team, where he also serves on the Primary Care Network executive. He previously chaired the Ontario Primary Care Council (October 2021– April 2023) and has held leadership roles with the OMA and South West LHIN. An adjunct professor in the Department of Family Medicine at Western University’s Schulich School of Medicine & Dentistry, he continues to advance primary care leadership and education. Dr. Schacter completed leadership programs at the Schulich School of Business and with the OMA/Joule, and in 2022 earned the Canadian Certified Physician Executive (CCPE) credential from the Canadian Society of Physician Leaders.
Dr. Marilyn Crabtree: Chair, June 2023-Oct 2024
Dr. Marilyn Crabtree is a family physician with 35 years’ experience in comprehensive, rural family medicine, including inpatient services, obstetrics, long-term care and palliative care. Her physician leadership has evolved over the past 15 years at the clinic and hospital level until shifting to system level primary care leadership at the Great River Ontario Health Team in 2021. Dr. Crabtree holds additional roles as a primary care leader in several regional and provincial programs in Ontario
Dr. Nicole Nitti: Chair, October 2024-Oct 2025
Dr. Nicole Nitti has been practicing comprehensive family medicine and acute care for over 25 years in a variety of settings including community health centres, office practice, shelters, long term care and emergency departments both rural and urban, working with diverse and marginalized populations. As her career progressed, she developed a passion for addressing social determinants of health and health inequities through primary care. Her leadership journey began in 2009 working with community and health care partners during the 2009 H1N1 Flu pandemic. Recognizing her aptness for leadership and system planning she began to take on leadership roles at local, regional and provincial levels. She currently has a comprehensive practice in West Toronto, is the Lead of the West Toronto Primary Care Network.
Outside of healthcare, Nicole is a mother of 3, an amateur musician, a marathon runner, and lover of anything fun.
Dr. Rosemarie Lall: Current Chair, November 2025
Dr. Rosemarie Lall is a respected family physician, health system leader, and digital health advocate with well over two decades of clinical and governance experience in Scarborough.
She is the inaugural Primary Care co-lead for the Scarborough OHT Primary Care Network
Some past leadership roles include: OPCC vice chair (2024), Co-Chair of the local Family Physician Network (SFPN). She is proud to be a current Board member at OntarioMD.
Dr. Lall brings a blend of clinical excellence, strategic foresight, and a deep commitment to collaborative leadership. Whether guiding future physicians, advising on provincial digital health initiatives, or leading system-wide change, she remains grounded in her belief that better tools and stronger partnerships lead to better patient care. She remains passionate about being a driving force in shaping a more connected, patient‑centred health system in Ontario.
Outside of healthcare, Rosemarie has lived and raised her family of 4 in Scarborough where she still resides with her spouse.
